How to get a water well in Duxbury Massachusetts

Here are the steps to getting a private water well in Duxbury Massachusetts:
1. Fill out a Water Well Construction Permit Application to the Duxbury Board of health. After you fill it out, you can mail it or bring it directly to the town hall at Duxbury Town Hall 878 Tremont St., Duxbury, MA 02332. There is a $100 fee. Here is a link to the Well Construction Permit Application: http://www.town.duxbury.ma.us/public_documents/duxburyma_health/FORMS/WELLCONST.PERMIT.pdf. The form will require you to name the Well Driller you will be using. You may put John McCue and then contact us at (781) 826-9235.
2. For a potable well, meaning one that will have water that is safe to drink, you must submit an Application for a Water Supply Certificate. Here is the link: http://www.town.duxbury.ma.us/public_documents/duxburyma_health/WELLINFO/WATER%20SUPPLY.pdf
3. Before a Water Supply Certificate can be issued, additional items must be submitted to the board of health including: a copy of the Well Completion Report, a copy of the pumping test report, a copy of the Water Quality Report, and an As-Built of the well location referenced to at least 2 permanent landmarks. 4.
